COMMUNITY SUPPORT WORKER

Job Summary:
This position is 30 hours per week, supporting individuals with diverse abilities within the
Greater Victoria and Sooke. Additional hours may become available in the future.
Ensure the well-being of the individuals and promote their growth and development. A
key component of this job is to support the individuals to learn the skills they need in
their physical, social, emotional and daily life skills development. The ultimate goal is to
increase their independence and support their success in the community.
Plan, develop, implement and evaluate activities with the individuals focusing on self-
determination, rights, emotional well-being, interpersonal relations, material well-being,
personal development, physical wellbeing and social inclusion.
The CSW strives to create a fun and enjoyable atmosphere in order to encourage
individuals’ participation.
Primary Duties:
Support individuals to learn the skills they need to allow them to function safely and
independently in their own homes and in the community.
Facilitate physical, recreational, social, employment and/or educational
opportunities, based on the needs and interests of the individuals.
Monitor individuals’ progress and well-being, and evaluate the effectiveness of
supports and services by keeping daily case notes of activities, progress towards
goals, and any other notable life circumstances that may affect their support needs.
Actively participate in and contribute to monthly staff meetings
Requirements:
• Community Support Worker Diploma or equivalent education, and
• At least two years previous work experience with people with diverse abilities in community settings.
• First Aid
• Crisis Intervention (SIVA preferred)
• Criminal Record Check to work with a vulnerable population
• Must own a reliable vehicle to transport individuals

HOME SHARE COORDINATOR (part -time)
Pay: CA$30.48-31.24 per hour
Job Summary:
This position is 10 hours per week, supporting Home Share Providers within the
Greater Victoria, Langford and Sooke area. Additional hours may be available in
the future should additional Home Share Providers be recruited.
The Home Share Coordinator is responsible for achieving the objectives of the Home
Share services, including: recruiting, screening, and selecting potential home share
providers; ongoing support and monitoring of existing home share providers; and
supporting the individuals in home share placements. The Home Share Coordinator
must be familiar with and in compliance with Community Living BC standards and
Building Bridges Community Support Policies and Procedures relating to home share.
Detailed records must be maintained using ShareVision and BrightHR.
Primary Duties:
Proactively recruit potential home share providers to ensure a sufficient
availability of placements for incoming referrals.
Manage the screening and selection process for applicants to become home
share providers in consultation with the Director.
Oversee a caseload of current home share providers, ensuring compliance
with CLBC home sharing standards and BBCS Policies and Procedures.
Schedule and implement quarterly monitoring visits with the home share
providers and the individuals in their homes.
Maintain organized, detailed, and confidential records.
Ensure that all documentation related to the home share providers are
maintained and updated as required.
Serve as a resource for individuals and home share providers, offering
support and assistance with problem-solving.

Qualifications:
- A degree or diploma in a related field such as Social Services, Human Services, or an equivalent area of study.
- Minimum of 2 years’ experience working with individuals with diverse abilities with at least 1 year working in home share
- Community Living Home Study Certification for Shared Living (adults)
- First Aid
- Crisis Intervention (SIVA preferred)
- Criminal Record Check
- Valid Unrestricted Class 5 BC Driver’s license Driver’s abstract
- Your own reliable vehicle
- Business insurance for vehicle, including $3,000,000 third party liability
