May 2 - Called income assistance to update plan c but couldn't get through, went to Dr Rhode office on cook st, breakwater walk, wears hat in sun, avoids sun, dislikes heat, do grocery shopping, meal planning, currently eats Raman noodles, likes bananas, Katrina came into her life at 8yrs old, july22 is her bday, loves Japanese food

May 7 - Called PWD office re: plan C & g, make Dr appointment in office, lifelab, change of address, need to buy lil sis Rosa bday gift, needs to buy phone minutes $25 per month, agreed to meet tues 9-3:30(half hr earlier than originally agreed upon)

May 9 -p/u biphentin; bi weekly pay, grocery shopping (called Katrina she etransferred $75), meal planning, talk w/Cecily about kitchen time (Cecily away due to surgery), dropped at boom&batten for sis bday, cooked chicken breasts and packaged individual portions and put in freezer

May 14 - -bus tickets -calendar for budget, pills, appointments, bdays, deferred college to march2025, supports are grow, youth empowerment society to achieve goals (cancel time with them?), called ministry of poverty reduction,
PHARMACARE ERROR IN SYSTEM, HE'LL PLACE PRIORITY REQUEST FOR UPDATE, THEY SHOULD CALL WED WITH UPDATE, put Katrina $52 reminder on calendar

May 16 -called PWD: Msp gender mismatch, gender provided to msp and phn aren't the same; phn isn't female; need to do change of gender designation form; contact msp, they need more info to go with the form, forms to do are section 2&4?, DID LAUNDRY, PRINTED GENDER FORM AT LIBRARY, DID CALENDAR, Monarch counseling 11:45 w/Erin may 29, boss asked if she can work on tues 18hrs per week 74 hrs per month, she doesn't want to, dad's bday this month

May 21 - biphentin 13+120 (20mg) 4/day,

Went to borden icbc, up mt tolmie, home to make icbc appnt online, Uploaded TEAM app for meeting, friend Zack lives esq walking distance, doesn't like wetness on hands or hair, plays one person games, bought msi console screen and keyboard on marketplace for $300, parents took her to Monarch in Vancouver for autism assessment (high functioning), raised $1000 through go fund me to pay for it, Maisie often avoids k's calls due to k being "aggressive" about wanting m to do things such as come over last night with a gift and card for dads bday, m didn't call or msg dad happy bday because feeling uncomfortable that she doesn't meet k's expectations, m currently doesn't have any money to buy gift/card, m enjoys using music software to create music online, m said she'll msg dad today to say she hopes he had a good day yesterday when dads done work

May 23 - 11:45 went to Dr Rhode appnt re:meds and blister pack, endocrinologist referral; completed msp gender change form, dr appoint, best buy, M wants to work at Tillicum rise weed store, grocery shopping

May 28 - attended 10am borden icbc appointment; Wrote letter to Emma at clbc, icbc gender designation form in, printed foi form at tillicum library and completed it, phoned ministry of poverty for $52 bus pass application

May 30 - 4-5:15pm zoom meeting with M's supports (see Nikki's minutes)

 

Complete paperwork and tasks that have built up over time, and that are causing Maisie anxiety.

By the end of this month, Maisie's anxiety level had reduced significantly. Although we completed many task, M still has many more goals to work on.
