July2. Had visit talked about going to Beacon Hill petting zoo. July 4.Rhonda received 50$ from social worker at summit for extra cigarettes .I went to smoke shop near her and picked them up..July6.had a visit was wanting to take Rhonda to Beacon Hill but she said want t feeling     well..July 8 took over 2 packs of cigarettes..July 8 got some lighters from $ store and took over as Rhonda said she needed some .then took over to her residence..July 13th Rhonda got 20$ from social worker for Cigarettes..she told me she gave the money to her partner who then spent it on alcohol..we have had conversations about not doing that as he cant be trusted..as i am there frequently I told her next time wait for me and I will get them. July 15th had a visit .we went out for a walk around the neighbourhood using her wheelchair..July 21st.took over cigarettes for Rhonda.July 25th got some supplies for Rhonda at $ store she needed.ie extra shampoo ..July 27th..Her social worker is trying to get more funding for cigarettes as the 4 paks she gets pr week frequetly runs out before the one week is up...

1. To make sure Rhonda is getting the support she needs in her residence 2. Encouraging to get out in the community and try new activities

Rhonda is frustrated with her partner as she says he is very controlling and doesnt  like her interacting with other people at the residence..she has met someone else(Man)  and I support her whatever decisions she makes around that. I keep reminding her to  careful and not let people manipulate her as they may take advantage of her good nature and not to share her cigarettes which she cannot afford to do. Overall the Summit residence has been a positive change for her and her hygiene has vastly improved .we will keep working on trying to get out into the community more and try new things
